Working Misunderstandings:Donors, Brokers and Villagers in Africa’s AIDS Industry

Why do development projects, and AIDS projects in particular, take the forms they do? In this essay we argue that it is because the conflicting interests and world views of the key actors involved—donors, brokers, and villagers—leave only a narrow range of themes and practices that can “work” on the ground. By “work” we do not mean, in the AIDS case, that they help prevent HIV transmission; indeed, the non-medical approaches to HIV prevention are largely ineffectual. This document is published by the Global Campaign for Microbicides and is based on interviews conducted in Cambodia and by telephone from mid-2006 through 2008 with key players in the events surrounding the 2004 trial to determine whether tenofovir disoproxil fumarate was safe and effective for use as a pre-exposure chemoprophylaxis to prevent HIV transmission. Before the trial formally began, preparations for it were halted by the Cambodian government following protests led by the Women’s Network for Unity (WNU), the union of Cambodian sex workers.

Since we first heard of HIV over 25 years ago, we have learned much about prevention. Knowledge about HIV transmission and the role of key interventions to prevent HIV transmission from mother-to-child and harm reduction initiatives for people who use drugs have dramatically altered the prevention landscape. However, a worrying trend has emerged from the shadows. The use of criminal statutes and criminal prosecutions against HIV transmission is an important human rights issue facing the world today.
